Passport Services

The City Clerk's office has suspended its passport service at this time. Please check other local muncilipalities or visit the U.S. Department of State Bureau of Consular affairs at http://www.travel.state.gov



    Passport fees if you are 16 years of age or older:    $75.00

    Passport fees if you are under 16 years of age:        $60.00

    Expedite fee is $60.00 per application

    Passport fees are paid to the U.S. Department of State by Check or Money Order only.

    Processing fee to the City of North Lauderdale is $25.00 per application (CASH ONLY)

It is a good idea to apply for your passport at least three months prior to your plan to travel. Passports take approximately 6 to 8 weeks with routine first class mail (2 weeks with expedited mail). Passports are valid for 10 years for adults; 5 years for children under 16.


WHAT TO BRING
 

Proof of Citizenship
An original or certified United States birth certificate, an original naturalization certificate, or an expired United States passport. (Documents must be submitted with application and will be returned to applicant with passport.)

Current Photo Identification
Driver’s license, state-issued identification card, expired United States passport, or other valid passport.

Passport Photos
Two identical, two-inch square photos in color or black and white. (Walgreen’s provides this passport photo service)

Fees
Check payable to the United States Department of State in the amount of $67.00 per application for adults and children over 16. Check in the amount of $52.00 to the U.S. Dept. of State for children under 16. Cash in the amount of $30.00 for the agency fee for processing the application. Expedited service is an additional $60.00 fee per application.

Special Requirements for Children
As directed by regulation 22CFR51- Effective February 1, 2004: Each minor child applying for a passport must appear in person.

As directed by Public Law 106-119 – Effective July 2, 2001: To submit an application for a child under age 14 both parents or the child's legal guardian(s) must appear and present the following:

Evidence of the child's U.S. Citizenship (or an original birth certificate or original naturalization certificate)
If the child has an expired passport – they will submit their expired passport along with their birth certificate
If only one parent appears you must submit the following: Consent form (DS 3053) signed by the non-attending parent and notarized Application of the applying parent must be signed in front of the clerk. Primary evidence of sole authority to apply or a written statement (made under penalty of perjury) explaining the second parent's unavailability.
